The $100 Billion Burden: Why Workplace Musculoskeletal Disorders Cost Far More Than Your P&L Shows

Every year, U.S. employers spend an estimated $58.78 billion on direct costs of workplace injuries — medical expenses, workers' compensation claims, and insurance premiums. That figure comes from the Liberty Mutual Workplace Safety Index, the most authoritative annual measure of employer injury costs.

But the direct costs are only the visible tip.

When you add indirect costs — lost productivity, overtime to cover absent workers, temporary staffing, training replacements, reduced quality from inexperienced workers, and the long-term health impacts of chronic conditions — the total economic burden of workplace injuries in the United States reaches approximately $100 billion annually.

And the largest single category within that number is musculoskeletal disorders.

Why the Full $100 Billion Figure Matters

The $100 billion figure matters because it changes the ROI calculus for ergonomic interventions.

A back surgery costs $40,000 to $80,000. A single episode of carpal tunnel surgery costs $15,000 to $30,000. A workplace exoskeleton that reduces shoulder strain by up to 50% — the Comau MATE-XT at $4,295 — costs less than one-tenth of a single shoulder surgery claim.

The Santa Clara University study on the MATE-XT showed a 22.67-point reduction in lower back exertion on the Borg CR-10 scale. Applied across a team of workers performing overhead tasks, one exoskeleton can prevent the cumulative strain that leads to a $55,000 shoulder claim.

The Trend Is Moving Against Employers

The frustrating reality is that employer costs from MSDs are rising even as the overall workplace injury rate declines. The Bureau of Labor Statistics reports that the workplace accident rate has dropped by roughly 40% over the past 25 years — a genuine success story. But during that same period, the cost per claim has risen by more than 30%, driven by rising healthcare costs, higher litigation expenses, and increasing claim severity.

The National Safety Council data shows that claim severity is increasing by approximately 6% per year. A claim that cost $30,000 in 2021 now costs nearly $48,000. At this trajectory, the average MSD claim will exceed $70,000 by 2030.

The International Picture

The United States is not alone. The European Agency for Safety and Health at Work estimates that MSDs cost EU economies €240 billion annually — approximately 2% of EU GDP. The United Kingdom reports £20.7 billion in workplace injury costs, with MSDs accounting for the largest share. A 2025 study in the International Journal of Surgery estimated the global burden of occupational MSDs at $2.1 trillion when including healthcare costs, productivity losses, and long-term disability.

Why This Matters:

For every 100 workers in a manufacturing or logistics operation, roughly 3 will experience a lost-time MSD this year. For an operation with 500 workers in high-risk roles, that is 15 lost-time injuries at an expected cost of $700,000 to $1.2 million. Ergonomic interventions — including exoskeleton deployment — consistently produce ROI of 3:1 to 15:1. NIOSH estimates that systematic ergonomic programs can save employers $9 to $23 billion annually in MSD-related costs. The math is not complicated.
